IS THERE A WAITING PERIOD FOR THIS PLAN?

There is a 6-month waiting period for implants. There are no other waiting periods.

PRE-EXISTING CLAUSES?

No, if you are in the middle of some work this plan will cover the work not yet completed. Your dentist would file for those services. This applies to orthodontia services as well.

WHAT IS THE DEDUCTIBLE FOR MY PLAN?

There is a $25 per individual deductible on the standard plan and a $50 per person deductible on the high plan. Preventative care services do not require a deductible.

DOES THE PLAN COVER BRACES?

Yes, for children and adults with the Premium Plan only. The high option plan provides an extra $2,000 benefit paid for orthodontia.

WHAT ARE THE BENEFITS OF THE HIGH OPTION PLAN?

The high option plan covers your preventative services and basic services (like the standard dental plan) and includes a higher annual maximum of $5,000 per calendar year. It will also cover your major services (i.e., extractions, root canal, bridges/crowns, etc) and offers the orthodontia coverage benefit.

WILL I GET AN ID CARD? WHAT DO I DO IF I’VE LOST MY ID CARD?

The Delta Dental insurance company mails out dental welcome letters and ID cards. If you need another card, just call us and we can mail another card.

CAN I ENROLL ANYTIME OF THE YEAR?

Yes, enrollment is an ongoing process. However, the claims are based on a calendar year basis. In January you start over with the annual maximum based upon the plan you elect – $1,250 for standard or $5,000 for the high plan.
