WHAT DOCTORS USE THIS INSURANCE? CAN I GO TO ANY VISION PROVIDER?

You may find a provider within the network and benefit only paying the $15 co-pay. To find a provider you may call the customer support service by calling 800-638-3120 or visit the website at www.spectera.com . You can search by city, zip code and name of provider.

WHAT IF MY PROVIDER IS NOT IN THE NETWORK?

You can have a Spectera representative contact your provider by nominating that provider. You may nominate your provider by using this form on the Spectera website or by calling the SEANC office. This is one way to increase participation of providers in your area.

WHAT IF I NEED TO FILE A CLAIM? HOW DO I GET REIMBURSED?

To request reimbursement, submit your receipts to:

The following information should be included with your itemized receipt submission:
Subscriber’s name and address
Member or patient's name and date of birth
Subscriber's unique identification number

CAN I ENROLL ANYTIME OF THE YEAR?

Yes, enrollment is an ongoing process. Your plan year will be based upon your effective date, which could be from July to July.
